Join our new webinar series to learn how communities can lead the effort to improve health care while lowering costs!

Brought to you by the Camden Coalition of Healthcare Providers and the PICO National Network, these online presentations will give community organizers, civic leaders and health care providers the building blocks to develop strong safety-net ACO's that improve health care and reduce disparities in their own communities. We'll share concrete ideas about how community organizing and "provider champions" can work together to align key stakeholders to reorganize local healthcare systems. (See schedule of events below).

The webinars will provide an introduction to Dr. Jeff Brenner and the Camden Coalition of Healthcare Provider's innovative campaign to give better care to vulnerable populations while driving down costs.  Dr. Brenner has been featured in the New Yorker and applauded by the Centers for Medicare and Medicaid Services. The series will also feature PICO training on how to engage patients and community, providers and elected officials to transform local health care systems from "sick care" to "health care". The PICO Network has over 30 years of experience driving community-led social reform on local, state and national levels.

This series will cover the key elements to build sustainable partnerships, create a community health database and deploy outreach teams focused on "high utilizing" patients, as well as how ACO's are structured for shared-savings and coordinated care. Open to all, the series will be of interest to community and faith leaders, patients, first-line providers, hospital executives and public health officials serving low and moderate-income communities.

The Camden Coalition of Healthcare Providers is a strategic initiative to improve the quality, capacity, and accessibility of the healthcare system for vulnerable populations in the City of Camden.  In this live presentation, Dr. Jeff Brenner will present an overview of the Coalition and its work in Camden, including healthcare issues faced by the state and City, the Camden Health Database, and the Coalition's initiatives to target specific patient populations.  The presentation will conclude with time designated for open Q & A. RSVP

The core of the Camden Care Management Project is a multidisciplinary outreach team, with skill sets needed to provide patient centered care to high utilizing patients with complex medical conditions and social histories.  In this live presentation, Kathleen Jackson and Kelly Craig will present an overview of the project, including roles and responsibilities of outreach team members, the referral process, intake and assessment activity, developing key ED/hospital-based partnerships, general process flows, challenges of the Camden-based project, as well as characteristics of high utilizers and lessons learned in providing care to this special patient population.  The presentation will conclude with time designated for open Q & A.RSVP

The PICO National Network is the largest grassroots organizing effort in the country, in over 150 cities and 17 states. PICO specializes in training vulnerable communities to lead social reform on local, state and national levels. Camden's success in aligning community and provider stakeholders was based in PICO's community organizing methodology. In this webinar, participants will learn how community members can engage with key stakeholders and decision makers in the healthcare system to address barriers to care and develop innovative structures for community input and governance that lay the foundation for a strong safety-net ACO. Since 2002 the Camden Coalition of Healthcare Providers has collected records on all admissions and emergency room visits from Camden's three hospitals for all Camden city residents.  Descriptive statistics and maps provide aggregate information to CCHP, the hospitals, payers and government organizations.  The results identify opportunities to deliver better, less expensive healthcare. In this live presentation, Dr. Ernest Post and Aaron Truchil will present an overview of how CCHP has utilized data to implement effective healthcare programs. Topics discussed will include data acquisition, data cleaning and standardization, probabilistic linkage and geocoding, and statistical applications. The presentation will conclude with time designated for open Q & A. RSVP
